A Quick Guide for Patients at Gas House Lane Surgery
If you would like to access an ADHD & ASD assessment through the NHS Right to Choose pathway, the process usually works as follows:
- Research your chosen provider
Patients are responsible for choosing the provider they wish to be referred to.
Different providers offer different services, waiting times and referral processes.
The practice cannot recommend a specific provider and may not have previous experience of the organisation you choose.
- Follow the provider’s instructions
Each Right to Choose provider has its own referral requirements.
Some providers:
- require patients to complete online forms or questionnaires;
- ask patients to download and return paperwork to the practice;
- require consent forms before a referral can be made;
- use online-only referral portals;
- have GP referral forms that must be completed separately.
Please carefully review the instructions on your chosen provider’s website before contacting the practice.
- Provide the required information to the practice
Once you have completed any patient forms or documentation required by the provider, please send this information to the surgery or advise us which provider you have chosen.
The practice can then review the information and arrange completion of any GP documentation required.
- GP review and referral
A GP may need to review your request before a referral is submitted.
Please note:
- referrals cannot usually be processed until all required documentation has been received;
- some providers have incomplete or unclear instructions, which can occasionally delay processing;
- the practice may need additional time to review unfamiliar providers or referral pathways.
Communication
The practice may contact you by SMS text message, NHS App messaging, telephone or letter.
Important
Choosing a Right to Choose provider does not automatically guarantee acceptance onto that provider’s pathway, ongoing treatment, or future shared care arrangements.
If you are unsure what your chosen provider requires, please contact the provider directly in the first instance.

If you are having difficulty completing the first stage of a Right to Choose referral for ADHD or Autism, please contact the practice.
We recognise that some patients may find online forms and referral processes challenging. Where needed, a member of our administrative team can arrange a time to sit with you in a private meeting room and help you complete your section of the referral form.
Please note that staff can provide practical assistance with navigating the referral process and completing forms, but cannot advise on clinical questions or determine eligibility for assessment.
